The Kingston PTA compiles an annual directory for use by the families. This directory includes each student’s name, parent’s name(s), address, phone number and email address. Please mark the appropriate boxes below and complete the requested information.
By submitting this form, you are giving consent to have your student's information printed in the directory. Please complete one form per Kingston student so that each child is included in the directory.
One directory will be given to each family with one or more PTA memberships. Additional directories are available for $1.00 each. Please send the dollar into your child's teacher in an envelope marked, "Extra Student Directory" with the child's name, teacher, and room number.
Student Artwork is also included in the directory. We invite all students to participate by submitting their artwork under these guidelines: Drawings should be black and white and no larger than 8.5” x 11” (smaller pictures are more likely to be included.)
Please submit this form, or return the printed form along with all artwork to your child’s teacher by Friday, September 16th, 2016.
